Used NINA for dithering with the mount and centering after every 3 frames to reduce drift.

– no calibration frames
I assume these black spots are dust/spots on my lens so i will need to clean this. Or could this be damage to my sensor?
Whats with the horizontal lines? Will flats or biases fix this?

It’s dust. Flats fix this. Always do calibration frames.
What bortle zone did you take this picture in? The horizontal lines are banding artifacts if I’m not wrong, you can fix them in postprocessing; in Siril there’s a filter to remove banding and it works great for me (I also use a Canon EOS 500d).
